You will also need to complete your US Lacrosse Association membership at www.uslacrosse.org. (The button here will take you directly to the U.S. Lacrosse website.) Players will not be allowed to step on the field until all their forms have been received and their membership with US Lacrosse has been verified. Thank you for your understanding!
